This application is for a clinic or prescriber opening an account with CRE8. For help with an existing account or patient prescription, call 888.224.5181.
Where do I sign the agreements?
The application states that signatures are required after submission. Follow the signing instructions provided by CRE8 to complete the agreements.
Can I request the catalog first?
Yes. You can request the formulary catalog. A catalog request is separate from an account application.
