What information does the request form need?
Please provide your first and last name, email address, phone number, and company or clinic name. All fields are required. Complete the request form above and our team will follow up personally.
Can I browse the catalog on the website?
Submit the request form and a CRE8 team member will contact you to share the catalog directly. The prescription formulary is not available to browse or download on the website.
Does a catalog request create a prescriber account?
No. Requesting the catalog is separate from opening a prescriber account. Start with our prescriber account application, or call 888.224.5181 for help.
How do I confirm a specific preparation?
Contact our pharmacists with the formulation name, dosage form and patient’s state. A catalog listing does not confirm current availability or suitability for an individual patient.
